Rehabilitation - Social Services and Discharge Planning
The unit’s licensed Social Worker can help patients and families in many ways:
- Emotional and psycho-social support to the patient and family while the patient is on the unit
- Assistance in securing medical equipment and/or post-hospital therapy appointments before the patient is discharged from Good Sam
- Connection with community resources to help families or friends care for the patient at home, including Meals on Wheels, Outreach transportation services, adult day health services and many others
- Participation in support groups, including support groups for stroke survivors and another group for care givers.
